Accident Summary:

 THE CREW OF UH-1H SN: ^68-16360^ WAS ON A MISSION TO INSERT A LONG RANGE RECONNAISSANCE PATROL TEAM AFTERWARDS TO PRACTICE SOME EMERGENCY PROCEDURES SO AS TO QUALIFY ^WO1 RODRIGUEZ^ AS AN AIRCRAFT COMMANDER. THE CREW CHIEF, ^ SP4 GREEN^ WAS ON THE RIGHT SIDE OF THE AIRCRAFT, BEHIND THE PILOT ^WO1 RODIGUEZ.^ ^SP4 BEEBE^ WAS ACTING IN THE CAPACITY OF A GROUND GUIDE IN FRONT OF THE AIRCRAFT, SO THE LEFT SIDE OF THE AIRCRAFT WAS OCCUPIED SOLELY BY CPT FARROW. AS THE AIRCRAFT WAS HOVERING FORWARD THE ROTOR WASH BECAME SO INTENSE THAT THE GROUND GUIDE HAD TO COVER HIS EYES. AS THE AIRCRAFT HOVERED TO WHAT WO1 RODRIGUEZ THOUGHT WAS CLEAR OF THE REVETMENT WALL HE ANNOUNCED "TAIL COMING LEFT", AND BEGAN TO SWING THE TAIL LEFT. ^CPT FARROW^ ATTEMPTED TO TELL HIM THAT THE TAIL WAS NOT CLEAR LEFT BUT THE TAIL ROTOR HAD ALREADY STRUCK THE REVETMENT. THE AIRCRAFT BEGAN A VIOLENT SPIN TO THE RIGHT, AT WHICH TIME ^CPT FARROW^ ROLLED OFF THE THROTTLE AND PUT THE AIRCRAFT ON THE GROUND. THE AIRCRAFT TURNED APPROXIMATELY 160 DEGREES AND CAME TO REST 40 FEET FROM THE REVETMENT WALL.\\
