Accident Summary:
ON 25 FEB 1970, TWO AH1G AIRCRAFT FROM A BATTERY, 4TH BATTALION, 77TH ARTILLERY (AERIAL ROCKET) WERE OPERATING NEAR THE AREA KNOWN AS THE RAZOR BACK ON A VISUAL RECONNAISSANCE MISSION WITH AN OBSERVATION AIRCRAFT (LOH). BOTH AIRCRAFT WERE FLYING COVER FOR THE RECON AIRCRAFT. ONE SHIP REMAINED HIGH WHILE THE SECOND ONE FLEW CLOSE COVER AT A LOWER ALTITUDE OF APPROX. 500-700' AGL. THE LOWER OF THE TWO AIRCRAFT WAS 67-15838. THE PILOT IN COMMAND OF THIS AIRCRAFT WAS WO1 ROBERT J. SULLIVAN. THIS AIRCRAFT WAS THE WING SHIP OF THE FLIGHT OF TWO ARA AIRCRAFT. IT WAS LOADED WITH 19 PAIR OF 10 POUND 2.75 FFAR, 11.5 PAIR OF 17 POUND 2.75 FFAR, 1500 ROUNDS 7.62 MM, 150 ROUNDS OF 40MM, AND 900 POUNDS OF FUEL. THE GROSS WEIGHT WAS 9393 POUNDS ON TAKE OFF. THE AIRCRAFT WAS OPERATING UNDER NORMAL CONDITIONS AT APPROXIMATELY 500-700' AGL WHEN THE ENGINE SUDDENLY FAILED. AN EYE WITNESS LOCATED ON THE GROUND BELOW THE AIRCRAFT STATED THAT HE HEARD A LOUD PUFFING NOISE AND AS HE LOOKED UP IN THE DIRECTION OF WHERE IT CAME FROM OBSERVED THE LOWER OF THE TWO COBRAS COMING DOWN AFTER A LARGE WHITE PUFF OF SMOKE CAME FROM THE EXHAUST OF THE AIRCRAFT FOLLOWED BY PIECES OF BURNING DEBRIS. WO SULLIVAN WHO WAS AT THE CONTROLS AT THE TIME REALIZED IMMEDIATELY THAT HE HAD EXPERIENCED AN ENGINE FAILURE AND HE PUT THE PITCH DOWN AND ROLLED OFF THROTTLE TO THE FLIGHT IDLE POSITION AS HE ENTERED AUTOROTATION. WO DAMM THE PILOT/GUNNER STATED THAT WO SULLIVAN HAD THE AIRCRAFT WELL UNDER CONTROL AND THAT THE ROTOR RPM WAS IN THE HIGH GREEN ALL THE WAY DOWN. WO DAMM STATED THAT HE LOCKED HIS INERTIAL REEL AND TALKED TO WO SULLIVAN ON THE WAY DOWN, BUT THAT NEITHER PILOT THOUGHT ABOUT JETTISONING THE WHINSTORES DURING DESCENT. WO SULLIVAN HAD TURNED THE AIRCRAFT APPROXIMATELY 45 TO THE LEFT TO ATTEMPT A POWER OFF LANDING TO A ROUGH SURFACED DIRT TOAD. AS THE AIRCRAFT BEGAN TO CLOSE ON THE ROAD WO SULLIVAN INITIATED A FLARE IN AN ATTEMPT TO TOUCH DOWN WITH AS LITTLE GROUND RUN AS POSSIBLE. THE SURFACE OF THE ROAD WAS UNEVEN AND A ROLLING TYPE TERRAIN SLOPING SLIGHTLY UPHILL IN THE TOUCHDOWN DIRECTION OF THE AIRCRAFT. WO SULLIVAN MISJUDGED THE HEIGHT ABOVE THE GROUND BECAUSE OF THIS AND PULLED PITCH SLIGHTLY HIGH. THE RESULTING DECAY IN ROTOR RPM WAS SUFFICIENT THAT THE AIRCRAFT FELL THROUGH THE LAST 5 FEET OR SO VERTICALLY WITH INSUFFICIENT RPM TO CUSHION IT ON THE GROUND. THE AIRCRAFT HIT HARD AND BOUNCED, COMING TO REST IN AN UPRIGHT POSITION OFF THE SIDE OF THE ROAD (PHOTO A & B). ON THE INITIAL IMPACT BOTH FORE AND AFT CROSSTUBES ON THE RIGHT SIDE FAILED AT THE POINT WHERE THEY WERE ATTACHED TO THE FUSELAGE CAUSING THE RIGHT SKID ASSEMBLY TO SEPARATE FROM THE AIRCRAFT (PHOTOS C & D). THE LEFT SKID ASSEMBLY REMAINED ATTACHED TO THE FUSELAGE BUT WAS BENT OUTWARD SO AS TO ALLOW THE UNDERSIDE OF THE FUSELAGE TO COME INTO CONTACT WITH THE GROUND. THE PICTURES TAKEN OF THIS AREA DID NOT DEVELOP. THE INBOARD SM-159 POD ON THE RIGHT SIDE SEPARATED FROM ITS BOMB SHACKLE ON IMPACT AND COLLIDED WITH THE RIGHT SKID ASSEMBLY WHICH WAS ALSO DEPARTING THE AIRCRAFT AT THAT TIME. THE AIRCRAFT CAME TO REST OFF THE ROAD SITTING ON ITS BELLY WITH THE RIGHT INB SM-159 POD JAMMED UNDER THE RIGHT OUTBOARD POD WHICH WAS STILL ATTACHED TO THE AIRCRAFT. THE RIGHT SKID ASSEMBLY WAS SMASHED AND PINNED BETWEEN THE RIGHT INBOARD POD AND THE GROUND (PHOTOS D & E). THE IMPACT OF THE FUSELAGE ON THE GROUND WAS SUFFICIENT TO WRINKLE THE TAIL BOOM IN TWO PLACES RESULTING IN TEARING A HOLE IN THE SKIN ON THE LEFT SIDE ALSO. DAMAGE TO TAILBOOM WAS AT STATION 358.0 ON THE RIGHT SIDE (PHOTOS D & G) AND AT STATION 352.0 ON THE LEFT SIDE (PHOTOS B & E). THE OVERALL TAILBOOM AND LOWER AFT HALF OF FUSELAGE APPEARED TO HAVE DAMAGE TO THE SUPERSTRUCTURE. THE FRONT GUNNER SIGHT DISMOUNTED AT THE BASE UPON IMPACT CAUSING SLIGHT INJURY TO WO DAMM LEFT LEG. THIS WAS THE ONLY INJURY THAT RESULTED FROM THE ACCIDENT. AFTER EVACUATING THE AIRCRAFT WO SULLIVAN AND WO DAMM NOTICED WHAT APPEARED TO BE A FIRE IN THE ENGINE. THEY IMMEDIATELY REMOVED THE ENGINE COWLING AND PARTICLE SEPARATOR AND EXPENDED THE FIRE EXTINGUISHER CONTENTS INTO THE ENGINE INTAKE THEREBY PUTTING OUT THE FIRE AND PREVENTING FURTHER DAMAGE TO THE AIRCRAFT.\\
