Accident Summary:

 THE AIRCRAFT DEPARTED ^CAMP EVANS, RVN^, 25 AUGUST 1970 HOURS ON A COMBAT MISSION IN THE VICINITY OF FIRE SUPPORT BASE ^O'REILLY^. THE AIRCRAFT HAD COMPLETED THE MISSION IN THIS AREA AND WAS RETURNING TO ^CAMP O'REILLY^. AT APPROXIMATELY 0840 HOURS THE AIRCRAFT WAS APPROACHING FIRE SUPPORT BASE ^JACK^, ^LOCATED 5 KM SW OF CAMP EVANS^. THE AIRCRAFT WAS AT 2000 FEET INDICATED, HEADING 090 DEGREES. THE AIRCRAFT COMMANDER RECEIVED LANDING INSTRUCTIONS FROM THE CAMP EVANS TOWER AND BEGAN A DESCENT. IMMEDIATELY AFTER ENTERING THE DESCENT, BOTH PILOTS OBSERVED A LOSS OF POWER AS INDICATED BY A DECREASE IN ENGINE NOISE AND A RAPID LOSS OR N1 AND ENGINE AND ROTOR RPM. THE AIRCRAFT COMMANDER ENTERED AUTOROTATION AND SELECTED A LANDING AREA, ALTITUDE AT THIS TIME WAS APPROXIMATELY 1500 AGL. THE PILOT ACTIVATED THE WING STORES JETTISON AND THE TWO OUTBOARD PODS AND THE RIGHT INBOARD POD LEFT THE AIRCRAFT. THE LEFT INBOARD POD FAILED TO JETTISON. THE AIRCRAFT WAS HEADED TOWARDS A SMOOTH GRASSY AREA WITH A FINAL APPROACH HEADING OF 135 DEGREES AND A NORMAL AUTOROTATIVE GLIDE WITH AN AIRSPEED OF 80 KNOTS. THE AIRCRAFT COMMANDER MADE AN INITIAL APPLICATION OF COLLECTIVE PITCH AT APPROXIMATELY 12 FEET ABOVE THE GROUND. THE AIRCRAFT YAWED TO THE LEFT AND STRUCK THE GROUND IN TAIL LOW ALTITUDE WHILE IN A SLIGHT BANK TO THE RIGHT. AT IMPACT THE RIGHT SKID COLLAPSED AND THE TAIL ROTOR SYSTEM SEPARATED AT THE 42 DEGREE GEARBOX, SEVERING THE UPPER PORTION OF THE VERTICAL FIN. AT IMPACT THE MAIN ROTOR STRUCK THE GROUND ON THE RIGHT SIDE OF THE AIRCRAFT. THE AIRCRAFT REMAINED UPRIGHT AND SLID APPROXIMATELY ONE FOOT TO THE RIGHT BEFORE COMING TO REST.\\

 AIRCRAFT COMMANDER HAD PREFLIGHTED THE AIRCRAFT AND WAS REPOSITIONING IT FROM THE MAINTENANCE REVETMENT AREA TO THE MISSION-READY AREA. HOWEVER, THE TAIL ROTOR DRIVESHAFT COWLING (5TH SECTION) HAD NOT BEEN SECURED AFTER PREFLIGHT WHICH RESULTED IN A 14 CRACK TO THE COWLING.\\
