Accident Summary:
AIRCRAFT AH-1G, SN 67-15635, WAS LOCATED AT QUANG TRI COMBAT BASE, RVN, ON 30 APRIL 1970. THE AIRCRAFT WAS ONE OF TWO COBRAS COMPRISING A LIGHT FIRE TEAM IN SUPPORT OF U.S. ELEMENTS IN THE VICINITY. THE CREW WAS ALERTED AT APPROXIMATELY 0220 HOURS FOR A FIRE SUPPORT MISSION. AT 0225 THE AIRCRAFT ENGINES HAD BEEN STARTED, AND CLEARANCE FOR TAKEOFF HAD BEEN RECEIVED BY THE TWO AIRCRAFT. WO1 FRENCH, WHO WAS FLYING IN THE LEAD AH-1G, OBTAINED HOVER INSTRUCTIONS AND TAKEOFF CLEARANCE FROM THE TOWER. A FLARE SHIP, UH-1H, ON THE SAME MISSION RECEIVED CLEARANCE TO FOLLOW THE TWO AH-1G'S FOR DEPARTURE. THE TWO AH-1G AIRCRAFT HOVERED FROM THE REVETMENTS DOWN THE PARALLEL TAXIWAY (HEADING 140) FOR APPROXIMATELY 200 FEET TO THE ACCESS TAXIWAY (ALPHA) FOR THE ACTIVE RUNWAY. THE LEAD AIRCRAFT, FLOWN BY WO1 FRENCH, ENTERED ONTO THE ACTIVE RUNWAY AND COMMENCED ITS TAKEOFF ON A HEADING OF 320 DEG. AS THE LEAD AIRCRAFT REACHED APPROXIMATELY THE MIDWAY POINT ALONG THE RUNWAY, THE SECOND AH-1G (67-15635) WAS HOVERED ONTO THE RUNWAY, AND CPT SMITH COMMENCED HIS TAKEOFF. AS THE SECOND AIRCRAFT APPROACHED TRANSLATIONAL LIFT, 100 FEET DOWN THE RUNWAY, CPT SMITH NOTICED THE RUNWAY MATTING RAISED IN AN ARC IMMEDIATELY TO HIS FRONT. THE HEIGHT OF THE MATTING WAS ABOUT FIVE FEET ABOVE THE RUNWAY SURFACE. THE AIRCRAFT IMPACTED WITH THE MATTING IMMEDIATELY. THE NOSE OF THE AIRCRAFT PITCHED DOWN. THE XM-28 SYSTEM AND THE INTERNAL AMMUNITION STORES WERE TORN FROM THE AIRCRAFT. THE ROTOR BLADES MADE CONTACT WITH THE MATTING SURFACE (SEE PHOTOGRAPHS 6-11 AND 6-12). THE AIRCRAFT HAD INITIALLY YAWED TO THE RIGHT UPON HITTING THE LOOSE MATERIAL. THE TAIL CONTINUED TO THE LEFT. THE AIRCRAFT STRUCK THE RUNWAY WITH THE LOWER LEFT PORTION OF THE NOSE SECTION, AND SETTLED ON ITS LEFT SIDE, COLLAPSING THE LEFT SKID. UPON FINAL IMPACT, THE ROTOR BLADES MADE CONTACT WITH THE TAIL BOOM AND SEVERED THE TAIL ROTOR DRIVE SHAFT. THE TRANSMISSION WAS TORN FROM THE AIRCRAFT UPON IMPACT, AND THE ROTOR HUB AND THE REMAINING PORTION OF THE BLADES WERE WRENCHED LOOSE.\\
