Accident Summary:
3. DESCRIPTION OF ACCIDENT AT 0930 HRS ON 15 JULY 1970,^WO1 OZENBAUGH,^ PILOT AND ^SP4 GARRATY,^ CREW CHIEF, TOOK OFF IN AH-1G HELICOPTER ^67-15604^ ON WHAT ^WO1 OZENBAUGH^ THOUGHT WAS A MISSION TO TEST FIRE THE AIRCRAFT WEAPONS SYSTEMS. AFTER TAKE OFF ^WO1 OZENBAUGH^ REPORTED HIS TAKE OFF TO HIS UNIT OPERATIONS AND WAS INFORMED THAT HE HAD THE WRONG AIRCRAFT AND TO RETURN AND TAKE THE OTHER AIRCRAFT. THIS ^WO1 OZENBAUGH^ STARTED TO DO AFTER FLIGHT CHECKING THE RADIOS ON ^67-15604.^ JUST PRIOR TO LANDING ^WO1 OZENBAUGH^ WAS INFORMED ANOTHER PILOT WOULD TEST FIRE THE OTHER AIRCRAFT. ^WO1 OZENBAUGH^ THEN REQUEST PERMISSION TO PRACTICE A GCA INSTRUMENT APPROACH. PERMISSION WAS GRANTED BY THE OPERATIONS OFFICER. THE AIRCRAFT WAS THEN FLOWN EASTTOWARD THE COAST AT APPROXIMATELY 1200 FEET AND 100-110 KNOTS. AFTER REACHING THE COAST APPROXIMATELY FIVE KILOMETERS NORTH-WEST OF EAGLE BEACH,^WO1 OZENBAUGH^ STARTED A DECENT AND HEADED SOUTH-EAST TOWARD EAGLE BEACH TO SHOW THE BEACH TO THE CREW CHIEF. ^WO1 OZENBAUGH^ DECENDED TO APPROXIMATELY 300FEET AND 110-120 KNOTS AND PARALLELED THE COAST LINE SOME 300 METERS FROM SHORE. AFTER PASSING EAGLE BEACH, ^WO1 OZENBAUGH^ STARTED CLIMBING RIGHT HAND TURN TO START BACK TO CAMP EVANS AND PRACTICE A GCA APPROACH, HE MADE A STEEP TURN AND WAS USING 33 POUNDS OF TORQUE. JUST PRIOR TO ^WO1 OZINBAUGH^ LEVELING THE AIRCRAFT BOTH CREW MEMBERS HEARD A LOUD EXPLOSION. ^WO1 OZENBAUGH^ IMMEDIATELY CHECKED HIS INSTRUMENTS AND SAW THAT HIS ROTOR RPM WAS DOWN TO 200. ^SP4 GARRATY^ NOTICED THE SAME THING AND ALSO THAT THE CHIP DETECTOR LIGHT HAD COME ON. ^WO1 OZENBAUGH^ REALIZED HE HAD ENGINE FAILURE AND IMMEDIATELU ENTERED AUTOROTATION. HE CONTINUED IN A RIGHT HANDTURN AND INITIARED A FLARE IN AN ATTEMPT TO GAIN ROTOR RPM. THE PILOT LEVELED THE AIRCRAFT WHEN HE WAS HEADING DIRECTLY FOR THE COASTLINE (330) AND NOTICED HIS ROTOR RPM IN THE HIGH GREEN RANGE. AT THIS TIME HE DETERMINED THE AIRCRAFT WOULD NOT MAKE THE SHORE LINE AS THE AIRCRAFT WAS APPROXIMATELY 60 FEET ABOVE THE WATER. ^WO1 OZENBAUGH^ THEN FLARED THE AIRCRAFT AND MADE A ZERO AIRSPEED LANDING. UPON HITTING THE WATER THE AIRCRAFT STABILIZED UPRIGHT FOR TWO OR THREE SEDONDS THEN GENTLY ROLLED OVER ON ITS LEFT SIDE. THE MAIN ROTOR WAS TURNING SLOWLY BY THIS TIME AND HIT THE WATER CAUSING NO DAMAGE. ^WO1 OZENBAUGH^ IMMEDIATELY OPENED HIS CANOPY DOOR EHICH WAS ON THE TIGHR SIDE OF THE AIRCRAFT AND FACING UP. IT TOOK HIM THREE ATTEMPTS TO GET THE DOOR TO STAY OPEN BEFORE HE REMEMBERED HE HAD TO TURN THE HANDLE TO LOCK THE DOOR IN THE OPEN POSITION. HE THEN EXITED THE AIRCRAFT AND NOTICED THAT THE CREW CHIEF COULD NOT GET OUT SINCE HIS CANOPY DOOR OPENED TO THE LEFT SIDE OF THE SHIP. ^WO1 OZENBAUGH^ THEN REACHED INTO THE COCKPIT AND GOT THE BREAKOUT KNIFE AND BRIKE ^SP4 GARRATY'S^ WINDOW AND HELPED HIM OUT OF THE AIRCRAFT. THE AIRCRAFT COMPLETELY SUBMERGED IN APPROXIMATELY 40-50 SECONDS. PRIOR TO HEARING THE LOUD EXPLOSION, THE AIRCRAFT WAS FLYING NORMALLY AND ALL INSTRUMENTS AND WARNING LIGHTS WERE TEADING NORMAL. DURING THE DECENT ^WO1 OZENBAUGH^ DID NOT HAVE TIME TO TURN OFF THE BATTERY, MAIN FUEL OR TO MAKE A MAY DAY CALL. THE BATTERY AND FUEL SWITCHES WERE NOT TURNED OFF AFTER LANDING.\\
