ON ^23 JANUARY 1969 WO MICHAEL GEOGHAGAN (W3159518)^ WAS THE AIRCRAFT COMMANDER OF AN ^AH-1G #67-15595^. AT ^0840 HOURS WO GEOGHAGAN^ DEPARTED THE ^RUTHLESS^ MAINTENANCE AREA FOR A TEST FLIGHT. THE TEST FLIGHT WAS TO COMPLETE THE REQUIREMENTS FOR THE INSTALLATION OF A 90 DEG GEARBOX. THE FRONT SEAT OF THE AIRCRAFT WAS OCCUPIED BY ^SP5 CHRISTOBAL CAUDILLO (US55987927)^, THE CREW CHIEF OF THE AIRCRAFT. AFTER ABOUT TEN MINUTES OF POWERED FLIGHT A "GROWLING" NOISE WAS REPORTED BY ^WO GEOGHGAN^ AND ^SP5 CAUDILLO^. ^WO GEOGHAGAN^ DECIDED TO FLY TO ^PLEI DO LIM^ AND CHECK OUT THE GROWLING NOISE. HE WAS AT ^3500' HEADING 090 DEG^ WHEN HE ROLLED THE THROTTLE TO FLIGHT IDLE TO SEE WHAT EFFECT THIS HAD ON THE NOISE. HE STATED THE NOISE QUIETED DOWN AND AT THIS TIME HE ROLLED THE THROTTLE BACK ON TO SET-UP A NORMAL APPROACH, THIS IS AT 250 TO 300 FEET ABOVE THE GROUND. AS POWER WAS APPLIED TO SLOW THE AIRCRAFT FROM AUTOROTATIVE RATE OF DESCENT TO A NORMAL APPROACH THERE WAS AN INSUFFICIENT LEFT PEDAL TO KEEP THE AIRCRAFT ALINED WITH THE STRIP. AS MORE POWER WAS APPLIED THE AIRCRAFT YAWED FURTHER TO THE RIGHT. AT 20 FEET ABOVE THE STRIP ^WO GEOGHAGAN^ AGAIN ENTERED AUTOROTATION. AS HE ROLLED OFF THROTTLE THE NOSE OF THE AIRCRAFT SWUNG LEFT BACK TO 090 DEG. ZERO NINE ZERO WAS THE HEADING THE AIRCRAFT TOUCHED DOWN WITH. THE AIRCRAFT FELL THROUGH AT THE BOTTOM OF THE AUTOROTATION, STRIKING THE GROUND FIRST WITH THE TAIL STINGER AND THEN THE SSKIDS. THE AIRCRAFT HIT WITH ENOUGH IMPACT TO SPREAD BOTH SKIDS AND CAUSE THE MAIN ROTOR TO FLEX DOWN AND SEVER THE TAIL ROTOR DRIVE SHAFT. THE AIRCRAFT THEN BOUNCED INTO THE AIR. AS THE AIRCRAFT MADE CONTACT WITH THE GROUND IT TURNED 120 DEG TO THE RIGHT ^(HEADING 210 DEG)^ AND CAME TO REST ON THE FUSELAGE. ^CW3 ROBERT KINLAW^ MADE A VISUAL INSPECTION OF THE TAIL ROTOR SYSTEM, FROM THE 42 DEG GEARBOX TO THE TAIL ROTOR AND FOUND NO MATERIAL OR MAINTENANCE FAILURE. THE CHIP DETECTOR WAS PULLED ON THE 90 DEG GEARBOX AND THE PARTICLES FOUND WERE A RESULT OF NORMAL WEAR. ^CW3 KINLAW^ IS THE MAINTENANCE OFFICER FOR ^C TROOP^.\\

AT 0700 HOURS ON 14 JAN., 71, ^BROWNING, JAMES L., 239-68-0185 CW2,^ ARRIVED AT THE ^D CO., 101ST AVN. BN.^ FLIGHT LINE AND PREFLIGHTED HIS AIRCRAFT FOR HIS ASSIGNED ESCORT/VISUAL RECONNAISSANCE MISSION. AT 11:30 HOURS HE RECEIVED A DETAILED BRIEFING FOR HIS FLIGHT. AT 1600 HE DEPARTED ON A VISUAL RECON MISSION WITH HIS OBSERVER, ^COTTRELL, L, 401-60-8818, SGT.^ DUT TO POOR WEATHER IN THE RECON ZONE HE WAS UNABLE TO GET INTO THE AREA. AT THIS TIME HE PROCEEDED TO A SPECIAL STRIKE ZONE TO TEST HIS WEAPONS SYSTEM. THE STRIKE ZONE WAS LOCATED APPROXIMATELY 29K'S SOUTH OF PHU BAI (YC9786). ONCE THERE, HE BEGAN MAKING TEST RUNS AND ON HIS THIRD PASS HIS MASTER CAUTION LIGHT CAME ON, HIS ENGINE FUEL PUMP LIGHT CAME ON, HE RECEIVED BOTH AUDIO AND VISUAL RPM WARNING SIGNALS, AND HIS N1 AND N2 BEGAN TO DROP. ^CW2 BROWNING^ THEN ENTERED AUTOROTATION, PLACED HIS GOVERNOR CONTROL IN THE MANUAL POSITION WITH NEGATIVE RESULTS. HE THEN TRIED AN AIR RESTART AND GOT NO RESULTS. DUE TO HIS LOW ALTITUDE OF APPROXIMATELY 900 FEET ABSOLUTE AT THE START OF THE MISHAP HE WAS NOW DANGEROUSLY CLOSE TO A RIDGE LINE AND HAD TO BLEED OFF ROTOR RPM TO LOW GREEN TO AVOID IT. AFTER CROSSING THE RIDGE LINE HE DECELERATED SLIGHTLY TO REBUILD RPM AND THEN CONTINUED AUTOROTATION. JUST SHORT OF HIS TOUCHDOWN POINT HE AGAIN HAD TO USE RPM TO AVOID LANDING IN A CREEK BED. ONCE ACROSS THE CREEK HE FLARED AND DESCENDED TO THE GROUND, LANDING HARD IN ELEPHANT GRASS ON TERRAIN SLOPED DOWN HILL FROM THE TAIL TO THE NOSE OF THE AIRCRAFT DOING DAMAGE TO THE TAIL BOOM, MAIN AND TAIL ROTOR BLADES, AND DAMAGE TO THE LANDING GEAR. HE THEN SHUT THE AIRCRAFT DOWN AND HE AND THE OBSERVER GOT OUT OF THE AIRCRAFT AND WERE PICKED UP BY AIRCRAFT AND TAKEN TO PHY BAI, CHECKED FOR INJURY AND RELEASED.\\
