Accident Summary:
AT ^2055H A SECTION OF ARA^ WAS SCRAMBLED ON A FIRE MISSION. THE START AND RUN UP OF THE AIRCRAFT WERE NORMAL. A PRE-TAKE OFF CHECK WAS MADE BY THE AIRCRAFT COMMANDER AND ACKNOWLEDGED BY THE GUNNER. THE EGT AT THE TIME OF TAKE OFF WAS ABOUT SIX HUNDRED DEGREES AND THE AIRCRAFT COMMANDER STATED HE WAS PULLING FROM THIRTY-EIGHT TO FORTY POUNDS OF TORQUE AT NINETY-SEVEN PERCENT N1. UPON TAKE-OFF A RIGHT CLIMBING TURN WAS MADE. AT AN ALTITUDE FROM 150 TO 200 FEET WITH AN INDICATED AIRSPEED OF FORTY TO FIFTY KNOTS, THE AIRCRAFT COMMANDER AND THE GUNNER HEARD A LOUD EXPLOSION FROM THE REAR OF THE AH-1G AND EXPERIENCED AN IMMEDIATE YAWING TO THE LEFT. THE IMMEDIATE REACTION OF THE AIRCRAFT COMMANDER WAS TO LOWER COLLECTIVE PITCH, AT WHICH TIME HE NOTICED THE ENGINE RPM GOING BELOW 6000 (295 ROTOR RPM). THE AIRCRAFT WAS IN A LEFT DESCENDING TURN. RIGHT PEDAL WAS APPLIED TO CORRECT THE LEFT YAWING TO NO AVAIL. THE AIRCRAFT COMMANDER ALSO STATED HE STARTED A SLOW DECELERATION AND THE YAW COULD NOT BE CORRECTED. AT TEN TO FIFTEEN FEET OFF THE GROUND THE AIRCRAFT COMMANDER APPLIED COLLECTIVE PITCH TO CORRECT THE YAWING. THIS DID NO GOOD WHATSOEVER. THE AIRCRAFT CONTACTED A TWO FOOT HIGH IRRIGATION DIKE AND ROLLED ON ITS' RIGHT SIDE, SEVERING THE TAIL BOOM AT IMPACT. AS THE AIRCRAFT CAME TO REST THE AIRCRAFT COMMANDER CRACKED OPEN THE LEFT SIDE OF THE CANOPY AND BOTH PILOTS SCRAMBLED AWAY FROM THE AIRCRAFT. THE AIRCRAFT DID NOT CATCH FIRE. BOTH PILOTS WERE IMMEDIATELY PICKED UP AND TAKEN TO THE ^45TH EVAC HOSPITAL^, THEN TO THE ^15TH MED^ FOR OBSERVATION AND LATER RELEASED TO DUTY.\\
