Accident Summary:
THE AIRCRAFT WAS FLYING THE WING SHIP FOR ANOTHER AIRCRAFT AT ABOUT 1645H ON 14 APRIL 1969 AND WAS IN THE PROCESS OF PUTTING IN A GUNSHIP STRIKE ON A TREE LINE NORTHWEST OF TRA VINH. ON THE PULLOUT OF THE THIRD PASS, ^CPT RIZZE^ EXPERIENCED A HIGH FREQUENCY VIBRATION IN HIS PEDALS AND A SHUDDER THROUGHOUT THE AIRCRAFT. HE CLIMBED TO ALTITUDE, NEUTRALIZED THE PEDALS, AND THEN CHECKED FOR PEDAL RESPONSE IN EACH DIRECTION. THE PEDALS DID RESPOND IN BOTH DIRECTIONS, HOWEVER WTIH RIGHT PEDAL APPLIED, THERE WAS ALSO A HIGH FREQUENCY VIBRATION. ^CPT RIZZE^ CALLED THE FIRE TEAM LEADER, AND THEY ELECTED TO RETURN TO THE TRA VINH AIRFIELD. ^CPT RIZZE^ PLANNED TO MAKE HIS APPROACH OVER THE ACTIVE RUNWAY AND PREPARED FOR A RUNNING LANDING IF IT WAS NECESSARY. HOWEVER, A UH-1C GUNSHIP WAS TAKING OFF 180 FROM ^CPT RIZZO'S^ LANDING DIRECTION AND SEVERAL UH-1H AIRCRAFT WERE HOVERING ACROSS THE ACTIVE RUNWAY TO THE POL POINTS. DUE TO THE FACT THAT HE STILL HAD PEDAL RESPONSE AND HE WAS NOT SURE WHAT WAS WRONG WITH THE AIRCRAFT, ^CPT RIZZO^ DECIDED NOT TO MAKE A GO-AROUND, BUT DECIDED TO MAKE A NORMAL APPROACH TO THE GROUND, OFF TO THE SIDE OF THE ACTIVE RUNWAY. THE APPROACH WAS FINE UNTIL THE AIRCRAFT CAME TO AN ALTITUDE OF ABOUT TWO FEET. AT THIS TIME ^CPT RIZZO^ BEGAN TO PULL A SMALL AMOUNT OF COLLECTIVE PITCH TO SLOW HIS DESCENT TO S SMOOTH, SOFT TOUCH DOWN. THE NORMAL TENDENCY OF A HELICOPTER IS TO YAW TO THE RIGHT WHEN COLLECTIVE PITCH IS INCREASED IN POWERED FLIGHT. AS ^CPT RIZZO^ INCREASED COLLECTIVE PITCH, THE AIRCRAFT YAWED TO THE RIGHT AND ^CPT RIZZO^ APPLIED LEFT PEDAL TO STOP THE YAW, BUT RECEIVED NO RESPONSE AS THE PEDAL SEEMED TO MUSH THROUGH, ^CPT RIZZO^ CEASED HIS APPROACH TO THE GROUND AND STOPPED AT A TWO FOOT HOVER. THE AIRCRAFT BEGAN TO DRIFT TO THE RIGHT REAR AND CONTINUED ITS YAW TO THE RIGHT. ^CPT RIZZO^ APPLIED FULL FORWARD, LEFT CYCLIC TO HALT HIS RIGHT REARWARD MOVEMENT AND DECIDED TO TRY AND ACCOMPLISH A HOVERING AUTOROTATION AT THIS POINT. AS THE THROTTLE OF A HELICOPTER IS DECREASED, THE NORMAL TENDENCY IS A YAW TO THE LEFT. PRIOR TO ROLLING OFF THE THROTTLE, ^CPT RIZZO^ MAINTAINED A SKIDS LEVEL ALTITUDE WITH FORWARD, LEFT CYCLIC. WHEN HE ROLLED OFF THE THROTTLE, THE AIRCRAFT STOPPED ITS RIGHT YAW BUT ^CPT RIZZO^ FAILED TO NEUTRALIZE THE CYCLIC CAUSING THE AIRCRAFT TO ROLL TO THE LEFT INSTEAD OF SETTLING IN A LEVEL ALTITUDE. AS ^CPT RIZZO^ LOOKED OUT TO THE LEFT, HE SAW THE MAIN ROTOR STRIKE THE GROUND. AT THIS POINT THE MAST SEPARATED APPROXIMATELY THREE INCHES BELOW THE MAIN ROTOR HEAD AND THE MAIN ROTOR CAME TO REST APPROXIMATELY 25 METERS FROM THE AIRCRAFT. THE AIRCRAFT CONTINUED TO ROLL TO THE LEFT AND CAME TO REST ON ITS LEFT TOP.\\
