Accident Summary:

AIRCRAFT WAS ASSIGNED A TWO-MINUTE HOT STATUS. THIS AIRCRAFT WAS LOADED WITH 30 PAIR OF 10 LB 2.75 FFAR, 1500 ROUNDS OF 7.62MM, 150 ROUNDS OF 40MM, AND APPROXIMATELY 850 LBS OF FUEL. THE WEIGHT OF THE AIRCRAFT AT TAKEOFF WAS 9000 LBS. AFTER BEING ASSIGNED TWO MINUTE HOT STATUS, CW2 BILLET WENT TO THE PARKING RAMP AND PREFLIGHTED HIS AIRCRAFT. DURING HIS PREFLIGHT HE NOTED THE FUEL GAUGE READING 600 LBS. BECAUSE OF THE MISSION REQUIREMENTS OF 1000 LBS OF FUEL, HE DECIDED TO FLY THE AIRCRAFT TO THE POL POINT AND REFUEL. SP5 VIRNIG, THE ASSIGNED CREWCHIEF WAS ABSENT SO PFC KARR ACCOMPANIED HIM TO ASSIST IN THE REFUELING OPERATION. CW2 BILLET DEPARTED THE ARA PARKING AREA TO THE NORTHEAST. DURING CLIMBOUT AND TRANSITION TO NORMAL CRUISE THE AIRCRAFT SYSTEMS GAVE NO INDICATION OF MALFUNCTION. JUST PRIOR TO REPORTING CROSSING THE ACTIVE RUNWAY CENTERLINE NORTH IN TRAFFIC FOR POL, AT APPROXIMATELY 300 FEET ALTITUDE AND 100 KNOTS AIRSPEED, CW2 BILLET NOTICED THE MASTER CAUTION LIGHT COME ON. HE HOOKED DOWN AT HIS CAUTION PANEL SEGMENT LIGHTS TO DETERMINE THE TYPE OF MALFUNCTION HE WAS EXPERIENCING. THE NUMBER 1 HUDRAULIC SYSTEM SEGMENT LIGHT FLICKERED AND WENT OUT ALONG WITH THE MASTER CAUTION LIGHT. WHEN CW2 BILLET RETURNED HIS VISUAL SCAN TO THE OUTSIDE AGAIN, HE FOUND THE AIRCRAFT IN A DIVING RIGHT TURN. HE APPLIED LEFT AND AFT CYCLIC TO CORRECT THE AIRCRAFT ATTITUDE AND PULLED COLLECTIVE PITCH IN AN ATTEMPT TO STOP THE DESCENT. THE AIRCRAFT STRUCK THE GROUND IN A SLIGHT RIGHT BANKING AND DECELERATING ATTITUDE. THE SKIDS SEPARATED FROM THE AIRCRAFT UPON IMPACT AND THE TRANSMISSION SUPPORT CASE FAILED ON THE RIGHT SIDE JUST FORWARD OF THE TRANSMISSION OIL INPUT CAP. AFTER THE SKIDS SEPARATED FROM THE AIRCRAFT THE XM-28 TURRET STRUCK THE GROUND WITH SUFFICIENT FORCE TO CAUSE THE TRANSMISSION TO TILT FORWARD AND THE PITCH CHANGE MADE CONTACT WITH THE FORWARD TOP SECTION OF THE FIBERGLASS TRANSMISSION COWLING. THE AIRCRAFT SLID ACROSS THE GROUND FOR 244 FEET LEAVING A TRAIL OF DEBRIS AND GROUND SCARS ALONG ITS PATH. DURING THE GROUND ALIDE THE HYDRAULIC LINES TO THE YAW CHANNEL SCAS ACTUATOR WERE RUPTURED CAUSING A FAILURE OF THE NUMBER ONE HYDRAULIC SYSTEM. THE AIRCRAFT BECAME AIRBORNE AND CW2 BILLET WAS ABLE TO REGAIN CONTROL OF IT. HE RETURNED TO CAMP EVANS AND LANDED THE AIRCRAFT WITHOUT FURTHER DAMAGE.\\
